Event Sponsorship Proposal

Bethpage Federal Credit Union requests everyone seeking Funding support, begin the process by reviewing our Eligibility Guideline for Heart of Bethpage Funding. Please keep in mind that if you are seeking Special Event Sponsorship, these events must support communities that lie within a three mile radius of one of our branches. Special Event Sponsorship consists of walks, runs, dinners, luncheons, breakfasts, festivals, concerts and any event in which Bethpage must purchase journal ads or tickets to attend. After reviewing the guidelines, if you are eligible please complete and submit the following:

All proposals will be reviewed by our committee. Please be sure to get your proposal to us two months in advance if there is a time frame. Only one proposal will be accepted per organization per calendar year. If you have received funding from Bethpage in the previous year(s), you must complete and submit the Outcome Assessment Questionnaire before applying for new funding.

When completing these forms, please fill in each required field. Then save the form to your computer and email completed form as an attachment to charitablegiving@bethpagefcu.com.
